The Freedom Care Program

Welcome to Freedom

Before moving into a Freedom Care Community, it is important to consider the costs involved for care. At Freedom, each individual will have the option to choose to join the Freedom Care Program (FCP) or be on a Fee-For-Service (FFS) user pays approach.

At every Freedom Care Community, every resident will be in their own home and will be encouraged and supported to live the way they choose.

What is the Freedom Care Program (FCP)?

The FCP is an optional program exclusively available to those who choose to live at a Freedom Care Community. Members of the FCP will have access to care and lifestyle services to help them remain independent in their Freedom home for the rest of their life, should they choose to, and it continues to be the best option for their wellbeing.

To ensure ongoing wellbeing, we work with each individual to fully understand their care, lifestyle and daily living needs. By getting to know them a little bit better, we can tailor precise care service requirements. We refer to these as ‘assessed care needs’. If there are additional services chosen or required, we can help with these too. They just have to be purchased in addition to the FCP fee.

The Freedom Care Program operates under the assumption that people enter the community with low to medium care needs, and over time, the need for more care may arise.

Early on, residents FCP fee may be more than the cost of their immediate assessed care need. In time though as their care needs change, their FCP fee may be less than the cost of their care. It’s a balance designed with certainty in mind going into the future.

If an individual has particularly high care needs at the time they join the FCP, their fees may include a needs supplement. This ensures that the cost of their individual care services doesn’t unreasonably impact the FCP for other residents.

Whether it’s the small things like watering pot plants or making a hot chocolate on a chilly night, Freedom’s model of care is based on the belief that most people prefer to live in their own home and want to remain as independent as possible.

The FCP compliments but does not replace Government Funded Home Care.  A Government Home Care Package does enable people with lower level care needs to continue living in their home, however this funding may not be sufficient if their care, lifestyle or social needs change. Freedom has a unique and innovative approach providing a complete range of care and support.

At Freedom, supplementing government funding (Home Care Packages) with the FCP is designed to enable residents to stay in the comfort of their own Freedom home in a social and welcoming community environment for as long as they choose to be there. The FCP operates on a cost recovery basis, meaning we are not profiting from it.

Our Freedom team will assist residents who wish to join the FCP to also access any and all government funding to which they are eligible and entitled to. Accessing and utilising Government Home Care Package funding is therefore a requirement of joining the FCP and ensures the program is not only fair and equitable, but also keeps the FCP fee as low as possible.

It is a requirement that residents entitled to government funded home care access this either as soon as reasonably possible after they enter a Freedom community, or as soon as reasonably possible after the need for funding and the ability to meet eligibility criteria emerges. Freedom staff are happy to assist. Veterans may also be eligible for DVA Community Nursing.

By joining the FCP, residents benefit from a range of care and support services, including:

  • Access to lifestyle and domestic services
  • 24/7 access to care services within their community
  • The opportunity to receive support for their assessed care needs immediately upon becoming a Freedom resident
  • The opportunity to receive assistance from Freedom community staff to access the government funding they are entitled to – this will enable them to supplement the services provided by the FCP
